This form must be completed by the person in charge (master) of the vessel prior to arrival in Australian territory, if intending to discharge ballast using other than Ballast Water Management Systems (BWMS). This form must be submitted to
Maritime NCC for assessment 96 to 12 hours prior to arrival as part of the pre-arrival documentation and reporting process.
