a Serious personal injury means that a person needs emergency treatment by a doctor; treatment in a hospital as a casualty, with or without being admitted to the hospital; or admission to hospital. b Prescribed incapacities of 30 days or more are not a required report under the WHS Act (from 1 January 2011). c The increase in the reporting of prescribed incapacities in 2010–11 resulted from improved and heightened awareness of the requirement to report incapacities. d Dangerous occurrences are 'near misses' that could have, but did not, result in death, serious personal injury or incapacity.
