This form is to be used by a vessel that is 7 metres or longer, and is on, or intends to make a voyage that arrives at, a place in Australian territory from either the Torres Strait protected zone or the Torres Strait permanent biosecurity monitoring zone.
This completed form must be provided to a biosecurity official or customs officer no more than 96 hours and no less than 12 hours prior to the vessel’s estimated arrival at the first destination port* on the Australian mainland. For further information please contact NAQS.
*For the purposes of the requirement above, a destination port is any point of landing for a vessel including, but not limited to, a boat ramp, beach or other geographic feature capable of harbouring a vessel, in addition to a designated port.
