Notification form for container and break bulk arrival/s

This form is to be used when notifying the department about the pending arrival of container and break bulk cargo. Please email or fax this form to your local department office for container and/or break-bulk discharge information at least 24 hours prior to vessel arrival on weekdays, and by 1400hrs on Fridays for weekend discharge.
