Integrity report form

We receive, triage, and assess all allegations of suspected misconduct, fraud and corruption, including referring complaints to another appropriate agency if required. You can use the form below to report suspected wrongdoing by departmental employees.

You can make a report anonymously. However, it may prevent us from contacting you to obtain further information, updating you on the progress of your report or limit our ability to resolve your report.

Describe the conduct you believe has occurred in as much detail as possible. For example, who was involved, what happened, where did it occur, when did it occur, why do you think it happened, and why do you believe it was inappropriate.
If you know please include the name, role, branch and division of any of the persons involved.

Privacy notice and consent

This notice explains how the sensitive personal information you provide to the department will be collected and handled as part of submitting your report to the Department of Agriculture, Fisheries and Forestry (the department).

Why are you receiving this notice?

You are receiving this notice because you are providing sensitive personal information to the department in the Integrity Report Form. The provision of this information will enable the department to appropriately address your report. We will use the information to assess, investigate and/or resolve your report.

What sensitive personal information will be collected?

The sensitive personal information you choose to provide in the Integrity Report Form about yourself, or other individuals, will be collected for the purposes outlined in the notice.

How will your sensitive personal information be used and disclosed?

The information you provide as part of this report, which may include sensitive personal information, may be used to assess, investigate and/or resolve your report. Your personal information may be disclosed to other areas within the department, other Australian government agencies, persons or organisations, provided the disclosure is consistent with relevant laws. Your sensitive personal information will be handled in accordance with the Privacy Act 1988 and our Privacy Policy, which is available on our website.

You can also choose not to provide your sensitive personal information.

However, this may mean the department will be unable to contact you to resolve your report and may be limited in the action it can take to address the report. It may also result in the matter being retained for the department's information only.

Your consent

This privacy notice explains how the sensitive personal information you provide may be handled by the department. If you have any questions or need help to complete the Integrity Report Form, please contact the department's integrity team on 1800 99 88 80, or via email at integrity@aff.gov.au.

If you decide to complete and submit an Integrity Report Form, you are taken to have consented to your personal information being collected, used and disclosed in accordance with this notice.
