Production questionnaire: Animal consumption and environmental purposes

September 2022

If you are importing plant-based goods for animal consumption and environmental purposes production to Australia, you will need to ask the facilities that manufacture and/or export your product to complete our production questionnaire.

Submit the completed questionnaire with your import permit application. If we advise you that an audit is required, you must also ask the facility to complete our audit questionnaire for plant-based stockfeed (also available on this page).

Production questionnaire: Third-party facility

The third-party facility questionnaire must be completed by the third-party facility where the goods are stored, handled or further processed. The questionnaire must be submitted with applications for permit to bring these goods into Australia. Manufacturers must provide information relating to the packaging of the product, as well as quality control measures in place at the facility. If the facility conduct additional manufacturing steps (e.g. Addition of materials, further refining) this questionnaire is not suitable, and the manufacturer must complete the Animal consumption and environmental purposes production questionnaire.

Submit the completed questionnaire with your import permit application.

Audit questionnaire: plant-based stockfeed

For completion by overseas facilities that manufacture, store, handle, transport or export processed plant-based stockfeed and stockfeed ingredients to Australia.

Application form: third party auditor

This form is to be completed by a potential third party auditor. We will use the information provided to assess their suitability to undertake site audits of overseas facilities used in the manufacture and export of processed plant-based stockfeed to Australia.
